The following documents include my Disclosure Statement as required by the Colorado Department of Regulatory Agencies, as well as documents that state my practice policies, practice agreements between myself and clients, consent forms and privacy practices (HIPAA) as required by federal and state laws. These documents can be downloaded and saved. Reviewing these documents and making a list of questions to discuss at our initial appointment is helpful. Prior to our initial appointment, these documents will be available to read and sign in my Client Portal. I will review the information contained in these documents at our initial visit and confirm that you understand and agree with my practices and policies.
